Permission to Publish

Consent and Release for Children and/or Adults(Required)

1. Permission to use Image and/or Voice Records Without Compensation. This consent and release is for past use and permission for present and future use for any lawful purposes of the video, photo, or voice recordings of individuals, including any minor child or children (“Image and/or Voice Records”) and is granted to Regina Angelorum Press Inc. and Ave Philomena, Inc. (“Released Parties”) free of charge and without any expectation of future payment from the Released Parties.
2. Consent and Release of Recorded Parties. This consent and release concern the individuals, including any minor child/children, listed in the fields below (the “Recorded Party/Parties”).
3. Waiver of Rights. Each undersigned waives any rights the undersigned, the Recorded Party/Parties, or any party related to the Recorded Party/Parties may have to inspect or approve the Released Party’s use of the Image and/or Voice Records for lawful purposes. The undersigned understands and agrees that the Image and/or Voice records will be available for viewing by the public without any manipulation of the media to disguise identity or facial features of the recorded parties, including the minor child or children, and there is no expectation of privacy concerning the use of the Image and/or Voice Records.
4. Indemnification. To the extent allowed by applicable law, the undersigned, on behalf of the undersigned, and the Recorded parties, shall indemnify the Released Parties for costs and expenses, including attorney fees, from any third-party claims or disputes arising from Released Parties use or possession of the Image and/or Voice Records. This indemnification includes any third-party claims alleging acts or omissions by Released Parties’ Board members, officers, directors, employees, volunteers, contractors and others acting under the permission or authority of Released Parties.
5. Release of Claims. The undersigned, on behalf of the undersigned, the Recorded parties, and any other parties related to the recorded parties, hereby releases the Released Parties and Released Parties’ Board members, officers, directors, employees, volunteers, contractors and others acting under the permission or authority of Released Parties from all claims relating to the Image and/or Voice Records (collectively, “Claims”),
